Switching is easy
You can switch from your current provider over to us.
In order to make your transition as seamless and efficient as possible, you must initiate this process by contacting your physician and/or previous supplies to acquire the documentation listed below:
1. Your prescription for CPAP/BIPAP Supplies and/or New Machine (if you are eligible)
2. Your original sleep studies (Diagnostic Polysomnography)
3. Chart notes from a recent office visit where your physician documents:
- Diagnosis of OSA
- Patient is compliant and benefiting from PAP Therapy
- Patient is benefiting from PAP use
That’s it!
Once we have received correspondence, we can continue working with you and your doctors to obtain all documents necessary to properly bill your insurance company.
